Thank you for your willingness to serve as a reference for a USF student. Simply download the appropriate form below. Use Form A if you observed the student in a non-teaching role and Form B if you observed the student teaching.

Please type your responses directly in the form. When you are finished, save it as a Word document and e-mail it to This e-mail address is being protected from spam bots, you need JavaScript enabled to view it . We will then bundle all the references into one document and forward them to the student as a PDF.
