Student Health Care
    USF has an agreement with the Center for Family Medicine to provide healthcare services to our students. However, students may use any of the medical providers in the Sioux Falls community and region. We encourage students to become established with a particular clinic so they get to know you and you in turn get to know them as your provider.

The Center for Family Medicine (CFM) is made available to USF students by calling the clinic at 339-1783.  The Center for Family Medicine is located at 1115 East 20th Street on the Avera-McKennan Hospital campus.  CFM welcomes you and their staff looks forward to meeting your medical needs.  

 Services to Students

 Any full-time USF Student may call the Center for Family Medicine to schedule an appointment at the CFM clinic. Prompt attention will be given to a student’s illness or injury and they will be seen in a timely manner. 

 Physician evaluations and treatment of USF students provided at CFM will be billed by the clinic to the student’s insurance plan. Students will be expected to pay the balance of the bill not paid by insurance. Proof of student’s health insurance is required at the first visit.

 Charges incurred from Reference Laboratory and/or Medical X-Ray Center will be billed directly to the student from those facilities.

 Physical exams will be available at a reduced rate for USF students at the cost of $30.00. Special group physicals will be available at $20.00 per student. Requested athletic physicals may be done at a cost of $20.00 each during the school year to also include physicals completed in August.

 CFM will perform TB testing for USF students in a practice teaching program and sign the appropriate “free of communicable disease” form.

 Clinic Hours:

 Monday – Thursday (8:30 – 5:30 pm and Friday (8:30 am – 5:00 pm). Center for Family Medicine is located at 1115 E. 20th Street. Physicians will be available to see students daily for acute care visits. On-Call service (after 5:30 p.m.) also available for USF students by calling the clinic @339-1783. www.centerforfamilymed.org.

 Emergency Care:

Students residing in a USF residential facility should contact a Resident Assistant, Assistant Resident Director or Resident Director who will contact emergency personnel to provide you with assistance.  Trauma Centers are located at both Sioux Valley Hospital and Avera McKennan Hospital in Sioux Falls. Students should take proof of health insurance with them. Students will be expected to pay the balance of the bill not paid by insurance.

Student Insurance:

 USF Students are required to carry health insurance.  An optional limited benefit medical plan providing additional benefits is made available to full-time USF students. Students are automatically charged $325 per semester for this limited benefit medical plan unless they complete the Benefits Waiver Form through the University indicating they are providing their own insurance coverage. Students wishing not to receive this optional coverage are required to complete the Waiver Form no later than September 17, 2010. The $325 charge will be removed from the student’s account once the Waiver Form is completed and filed with the USF Student Development Office (Student Center – lower level). Students needing a Waiver Form may obtain one in the Students Accounts Office or the Student Development Office (Student Center – lower level).
